Documents Checklist to Show Your Adoption Attorney

If you want to adopt a child, you will have to provide many documents and complete many forms. You will also have to retain copies of all the documents and forms you have submitted to the agency, the court and other authorities. The following list though not all inclusive will give you an idea of the documents you should provide your adoption attorney to put the process in motion. If you are providing copies of any official document, make sure you provide the certified copy.

• Copy of the adoptive parents’ birth certificate
• Copy of the adoptive parents’ marriage certificate
• Copy of divorce decree(s), if any
• Physical examination forms completed by a doctor
• Statement from the adoptive parents’ medical insurance company that the child will be covered under the policy
• Forms authorizing background checks
• Letter of good standing from the local police
• Social service agency release forms and reports
• Copy of passports of the adoptive parents
• USCIS Forms
• Home Study Report
• Reference Letters
